Please note, submission of a completed booking form constitutes a firm booking and as such the client agrees to Upkeep Training terms and conditions and will be liable for the applicable fees.

Please complete the table below for all delegates you wish to book on an Upkeep course(s). If the learner is the same as the booker, please write 'as above' in the box below. Please notify us in advance if you have any special requirements, i.e. access, learning support or dietary.

SECTION 3: Payment

When we have received your booking we will issue an invoice to you for payment. Please note that payment must be made in full prior to the course taking place.  VAT will be added as appropriate to your invoice.
